It's known throughout the games and novels that Philippa prefers women. In The Witcher 2: Assassins of Kings, there are 2 possible cutscenes where she's with her lover, Cynthia.
Whether you save Triss or help Philippa has no impact on how you eventually choose to deal with Sile and with Letho. Also, the way I see it, the choice is really whether to help Triss or to help Saskia (by way of helping Philippa).
Show it to Triss: If you have not yet completed the Now or Never quest, you can head back to Novigrad and meet up with Triss. This will make her very happy and reveal a hidden message about Philippa's quest. After showing it to her, you can decide whether you want her to keep it or bring it to Radovid.
If you don't give it to Triss, it will appear that the only option is to give it to Radovid. But if you hold on to the crystal until you finish The Last Wish, you will get the option to give it to Yennefer, even if the quest has already ended.
Geralt can either insist on going to the meeting with her or tell her she'll be fine and let her go alone. Players need to allow Ciri to go alone if they want this choice to count towards a positive ending. Going with you. You'll do fine on your own.

Voleth Meir, also known as the Deathless Mother, is a major antagonist in the Netflix television adaptation of The Witcher, serving as an unseen antagonist in season one, and one of the two main antagonists (alongside Rience) in season two.
The Emperor of Nilfgaard, aka the White Flame, was revealed to be Duny (Bart Edwards), Ciri's (Freya Allan) father, who was allegedly lost at sea with Ciri's mother when the princess was just a baby.
Voleth Meir, also known as the Deathless Mother, delivers on both fronts. Many of Sapkowski's creatures are inspired by folklore, and Schmidt Hissrich said Voleth Meir is based on the mythical witch Baba Yaga.
Bart Edwards portrays Duny, who we now know is Emhyr van Emreis, the emperor of Nilfgaard and father to Ciri. This explains why Emhyr is also joining the battle to find Ciri, as she is his own daughter.
Now, Ciri, given her family background, has Elder Blood, which grants her powers, and it was prophesized that her child would be the ruler of the world. This means that Nilfgaard is so desperate to catch Ciri because Emhyr wants her to bear his child, something Calanthe would have never approved of.
She brought in the elves to fight for Nilfgaard, but they'd had a change of heart. Blood being a bond stronger than all over, she pleaded with her uncle for help. He admitted that he should've stood up for her during the ball that night at Aretuza. Whatever was brewing, the Brotherhood needed reinforcement.
Sadly, Yennefer was left fuming and believed she didn't love Geralt and it was simply the djinn's magic which was behind it. Yennefer is said to be Geralt's true love, so it would suggest their feelings are genuine. Saying this, their romance will be a tumultuous one throughout The Witcher stories.
But Yennefer had another lover, the sorcerer Istredd, who she could not bear to part with. In the end Yennefer broke off her relationships with both men.
Who was Yennefer's first love?
Istredd was a sorcerer who was romantically involved with Yennefer during their time at Aretuza.
The outcome of the quest is basically the same, no matter what choices you make: Triss and Geralt kill Menge and his mooks; Triss is fine and won't hold it against Geralt; you learn where Dandelion is. The choices you make during this quest will not have much effect on future encounters with Triss.
Make sure you don't give it to his men, they trick you and you get no reward at all. Have Triss look at it first so you can see what's on it, then give it to Radovid for the gold. Besides, Phillipa is a terrible person and if any sorceress needs to be caught it's her.
' Your interrogation of Ciaran, the prisoner on the barge, will activate the story quest and you can either accept the offer Triss makes to come along with you or decline it. Though the events that follow differ slightly depending on your answer, there's really no good reason not to accept her help.
If you convince Keira to go to Kaer Morhen, she will join you for The Battle of Kaer Morhen. If she doesn't go to Kaer Morhen, the Brothers In Arms: Velen quest later will automatically fail. If you let Keira go to Radovid (or didn't do the quest at all), this will later unlock the secondary quest A Final Kindness.
However, it's also important to save the mages in Novigrad, to help kill Radovid, and finally, side with Vernon Roche and kill Dijkstra. If you don't do any of these things, Radovid will win and Ciri will become a witcher instead.
Before the play, you get to choose who will play the Princess and the Prince. Choose Priscilla for the role of princess and Aleblard for the role of the Prince. They do the best job and lead to the most successful performance. When the play starts, Geralt will play the role of the Witcher.
Regardless of what you choose to do with the crystal, you must go to see Radovid at the end of the quest. If Geralt has the item in his possession, he can give it to the King to receive an award. But if he's given it to Triss, Yennefer, the Witch Hunters, or he just keeps it to himself, then Radovid will snub him.
IIRC outcome of Redania's Most Wanted does not affect Reason of State (Radovid's assasination), but it is one of the quests that need to be completed for RoS to trigger.
You can choose either to spare Karadin or help Lambert kill him. Lambert will be angry with Geralt if you insist on leaving Karadin, but there will be no real consequences to the quest as it will not impact Geralt's future interacations with Lambert. Regardless of the choice, the two will go on their separate ways.
Can you romance as Ciri?
Ciri also has some romance options in the new game, and it's up to you to choose who she'll end up with. So, buckle up and check out your options – and how to achieve them.
Despite her powers, Ciri hasn't undergone the Trial of Grasses, so she doesn't have the enhanced physical prowess or longevity of a Witcher. Being an empress is a safer option for Ciri, and the comforts, protection, and amenities such a lifestyle provides means she'll probably live a far longer time.
- Bring Ciri to the Emperor. When Ciri suggests visiting Bald Mountain during Blood on the Battlefield, say "Gotta visit the Emperor first", not "Velen, then".

Ciri is the de facto strongest character in the Witcher. Even though you might think the book is mostly about Geralt, or maybe even Yennefer, in reality the whole story is about Ciri.
Voleth Meir persuaded Francesca to form an alliance with Nilfgaard, and she did so to protect her people, including her baby with Filavandrel — the first full-blooded elf to be born in years.
What did the deathless mother want with Ciri?
The Deathless Mother offers gifts in exchange for obedience or favors, but what does she want? The answer is almost too easy. She only wants to return to the otherworldly plane that she came from in the first place. Although it hasn't been completely revealed yet, this is why she's after Ciri.
She sent Yen hunting for Ciri because she knew of her power to break monoliths and teleport between realms—basically, Ciri's power was the only way she knew to get home.
